One of El Rubio's guards has apparently brought his island security standards back to the mainland and passed out in public. The Sleeping Guard can appear at one of 10 locations across Los Santos and Blaine County — search him and take the Small Key. Bring it to Cayo Perico during the Heist, unlock the desk drawer in El Rubio's office, and walk away with the Perico Pistol.
